Similar to #73, fix broken links and missing images in the German (and sometimes Catalan) translation.
This PR is quite a large, but most of the changes are trivial Markdown fixes. I have also tried to group them into meaningful commits.
This is especially important because the creation of localised PDF documents is going to be enabled by #81 and the DE version looks very machine-translated to me - which is of course fine, but also introduced a number of broken/hereby corrected Markdown links.
