Add VS Code extension for inline warnings #6

Developed VS Code extension according to requirements:

1. Integrated with isnad-scan to automatically scan open/saved files
2. Implemented inline warning decorations using VS Code Diagnostics API
3. Added hover tooltips with risk details and ISNAD finding information
4. Added configuration options for scan sensitivity, scan on open/save
5. Status bar integration showing current file risk level
6. Manual scan command for on-demand scanning

• Tested thoroughly against sample malicious files: all acceptance criteria met
• Deliverables:
  • Complete VS Code extension source code in
  • Test cases:
  • README and CHANGELOG included for marketplace publishing
