This is a rework of the files updated in PR #579

The logos in the odt files in PR579 were copied into the prior versions of the odt files, and then exported.

AFAICT this has fixed the ODT version issue and the check box alignment.

The fonts are different in the two files, but that is how they are now.

I just noticed that membership application says:

"and email the pdf to to secretary@apache.org"

Note the spurious 'to'.
Also it probably ought to clarify that the Postal Address needs to be a home address, not a work address.

Are there any more edits that need to be made?

It might be worth changing the title of the withdrawal-request to:

'Membership withdrawal request' to align with the application form.

Thanks for following up on this.
The withdrawal request form looks good as is.
The membership application form needs two small changes:
change Postal to Residence
change Telephone to Apache id

1. The name change to membership-withdrawal-request makes total sense.
2. The changes to the text in withdrawal and application are good
3. Go For It.
