Improving Accessibility of Ruby Annotations

This probably is a larger conversation with sub-issues, but I wanted to at least start it here. The current handling of ruby markup in UAs is very poor, and we should create guidelines and possibly additional markup support for doing better.

In particular, this comment details the majority of what is needed to alleviate the Ruby ambiguities that are currently causing problems for speech-based assistive technology like screen readers.
