Primary key for `RecipeIngredient` should not be just `[Recipe, Ingredient]`

Recipes can have more than one entry for the same ingredient, such as with sugar in both the filling and frosting of a cake.

With RecipeIngredient's primary key being [Recipe, Ingredient], this is currently unsupported.
