[Feature]: Add comments thread to Actions #326

Suggestion / Feature Request
As a coach or a coachee, I'd like to be able to have a comment thread between each other about an action that I'm working on so that:
- I can maintain more context associated with the action that comes up as new thoughts or hurdles to its completion
