[Bug]: "private struct TranscriptAnalysis {"

Steps to reproduce

I'm not completely certain - I wish I could upload logs of some sort. I was using Codex, GPT 5.4, high, full access, local.

I had made a plan - and codex was executing the plan.

I got a red error banner at the top of the chat "private struct TranscriptAnalysis {"

When I dismissed the prompt it returned almost immediately during the run.

The prompt disappeared on its own after the run ended

it did not seem to impact the models output at all and the run finished as expected.

Expected behavior

there should be no error - can be concerning for users

I'm seeing a similar thing consistently today. The most obvious trigger was doing a brew upgrade - that updated the Codex CLI to v0.117.0 and now every Chat GPT 5.4 thread I have running (I've got about 5 going) is occasionally spitting out lines from the code it is writing as error messages.

The errors themselves are harmless - as far as I can tell the actual code that is being written is fine. However when this happens it breaks the activity tracking in the sidebar, immediately showing the thread as completed despite it still actively working.. makes it hard to see when things have actually finished.
