Is it always necessary to use U+2146 in expressions like "d/dx"?

It seems to me that books often use Latin d. In addition, I think the reading of the expression is better with a Latin d.

Fair enough, this recommendation might have outlived its purpose. I haven't noticed problems with a regular d either, so especially if it reads better we should remove it.

Make a new example for integral, using <msubsup> and latin d in "dx". See discussion in issue #77

@samimaattaCelia, I see you have changed the example with integral to use <msubsup>. Can I expand that example to include the correct markup of "dx"?
