"Out of workspace" detection fails

Type: Bug

Extension assumes all files not opened using internal file explorer as "out of workspace". Which is clearly not true.

Also, it does not work well with pinned files. (You should not touch them!)

I'm working on a new implementation using the idea of #1, I think this would solve the problem.
