Many issues resolved, those remaining are flaky

https://github.com/mark-wiemer/mocha-examples-2/actions shows some passing and failing runs on the same code, demonstrating flakiness

Relatively unclear exactly what the issue is, there may be multiple issues or it may be the same issue reported differently in different versions of Node
