Combine the process of vendor and carver registration into one form

A user in the past has to register twice if they want to be a vendor and a carver. This should be combined into one registration process to reduce what they need to do,

The registrant might want to do the following:

• register as a vendor
• register as a carver
• register as a vendor and a carver
