Feature: Table of Contents Generation

This is a feature request: I'd like to be able to generate a table of contents at the start of a a slide deck. I imagine something like <!-- toc -->. I am not sure, though, if that should be based on Setext or ATX headings. Setext is probably too shallow with only 2 levels. ATX allow deeper structures but presenterm seems to opinionated on how to use them. Personally, I would forget about Setext and use ATX only to model a full document structure.

What would the ToC contain exactly? Slide titles are too granular IMO. e.g. if you have a slide deck with 20 slides you don't want a ToC with 20 items in it. The agenda at the beginning of a slide deck is generally a very high level overview of what will be talked about.
