Use expanded codes/value sets to generate the FHIR codes?

Currently it uses incomplete CodeSystems from the definition, it is better to use the expanded value sets.

Hmm maybe not actually that great, as the expanded codes are only the value sets and not expandable codes I think. Unsure if it is more codes. Also the expanded valuesets don't have a definition for the values.
