Create templates for contributions

Better documentation is needed assist contributors to the CCO GitHub repository.
@oliviahobai has volunteered to provide an initial list of templates, such as:

• How to create a new term request?
• How to create a pull request?
• How to format an issue?

Future tasking will involve creating drafts of these templates and expanding the list of available templates.

Instructions for Creating a New Term Request

1. Navigate to the [Issues] tab.
2. On the top right, select “New issue”.
3. Title the issue with the format: “New Term: [name of the proposed term]”
4. Describe the scope of the term and any reasoning for its inclusion as clearly as possible.
5. As best you can, include initial proposals for:
   a. Rdfs:label
   b. Definition
   c. Source
   d. Parent class

Note related terminology and state why those terms are inadequate or how they relate to the new term.
Be explicit about which ontology file the new term will be added to.

Next steps are to create a PR with a .md file containing these instructions in the "Contributing" section.

@APCox Re the third bullet, under the .github repository you'll find templates for issues which can be customized so that when users go to create an issue they can choose from a dropdown menu that is preformatted. Best to format issues for users ahead of time if we can.
